On reflection, I've decided to remove all the "hash tables as procedures"
interfaces except `hash-table-accessor` and `hash-table-accessor/default`
on the grounds of insufficient utility (as several people have urged
me to).  Only the accessors make sense in terms of the stated goal:
to be able to map a list, vector, etc. using a hash table to encode a
mapping function.
